Together Blue Medicare HMO is a Medicare Advantage Health Maintenance Organization that offers affordable plans made possible by partnerships with AHN community hospitals. Together Blue Medicare HMO includes a high-value network of AHN providers, PLUS an enhanced service model to assist in finding doctors, making medical appointments, and coordinating your health information between doctors and hospitals.

Not all providers will accept Together Blue Medicare HMO. Please verify that your providers are participating before enrolling. If a provider does not participate, neither Medicare nor Together Blue Medicare HMO will be responsible for the costs. 

Together Blue Medicare HMO is a limited network plan. If you want access to Highmark’s full provider network, including UPMC hospitals and physicians, you may wish to consider our Security Blue HMO-POS and Freedom Blue PPO Medicare Advantage products.  

Highmark Choice Company is an HMO plan with a Medicare contract. Enrollment in Highmark Choice Company depends on contract renewal.
