Your team will work to make sure your personal health needs are being met, including:

Case management:
A case manager can help coordinate your care and provide ongoing support for you and your family.

Disease management:
A registered nurse will help you better understand your condition, treatment options, and help you manage it so that you can live your best life.

Care transitions:
If you have a hospital stay, we’ll make sure you understand all your discharge instructions, help set up follow-up appointments, and make sure that any equipment you need at home is there waiting for you.

AIS Home Visit Program:
When needed, your Clinical Care Team can connect you with our Advanced Illness Service (AIS) Home Visit Program. This Highmark program offers extra in-home support to help you and your family when you are facing a serious medical condition.

Medication Therapy Management:
Your Clinical Care Team pharmacist works with your prescribing doctors to ensure that your medication regimen is safe, effective, and affordable. A pharmacist on your team can also help you:

— Review your prescription and over-the-counter medications, including vitamins and supplements

— Answer questions

— Discuss possible side effects

— Check for potentially harmful interactions between medications or supplements

— Explore options for lower-cost medication alternatives

— Provide a personal medication record for you and your prescribing doctors
