Preventive care is an important part of staying healthy. That’s why we’re making it easier for you to schedule time, once a year, with a medical professional to visit you in the comfort and privacy of your own home.

We’ve made it easier than ever for you to get a free health check. And the best part? You don’t even have to leave the house.

With your Highmark Medicare Advantage plan, you can schedule a free House Call Visit — bringing a licensed health professional right to your door. All licensed professionals are prescreened for COVID-19 before the House Call, but if you’d prefer, you can also complete the visit via telehealth, using a device with audio and video capabilities.

During this visit, you can get:

— A thorough health assessment from the comfort and safety of your own home, either in person or via telehealth.

— Quality one-on-one time from a licensed medical professional who can answer all of your health-related questions.

— An opportunity to review medications or discuss any new medical issues or concerns.

— A personalized health summary of your visit, which is also sent to your primary care physician.

Highmark House Calls may also count toward the Highmark Wellness Rewards program.

House Calls do not replace your annual wellness visit. These visits are designed to supplement the care you get from your primary care physician.

Members living outside of PA, WV or OH
Call Signify Health at 1-833-423-7925, Monday – Friday, 8 a.m. – 8 p.m. (TTY users call 711) to schedule your in-person or telehealth visit.
